How To Fill Out Workplace Injury WSIB Paperwork

What To Do If You’ve Suffered A Work Injury and WSIB

Tell your employer and fill out appropriate WSIB forms. Your employer should fill out WSIB FORM 7 and submit it to WSIB within 5 days of the reported injury. WSIB FORM 8 will be filled out by a medical practitioner (Chiropractor, PT, Medical Doctor). The patient with the work place injury should be completing a WSIB FORM 6 either at work or upon initial visit at a medical clinic. If you come to our Oakville WSIB clinic for treatment, we will help you with all forms needed for WSIB.

Seek treatment immediately. If you have to leave work early due to pain call 905-827-4197 and book an initial assessment and treatment with our Oakville WSIB chiropractor. The sooner you get in to see us, the sooner WSIB will approve your claim and the sooner you will heal.

If you need time off of work due to pain from your work injury, Nottinghill Family Wellness Centre can take care of this. Once approved from WSIB, if our chiropractor recommends time off from work, you should be compensated and proper documentation will be noted.

What’s Included in a WSIB Treatment Visit in Oakville

  2. Shockwave Therapy – Shockwave is a air powered gun that breaks up deep scar tissue and promotes healing by increasing blood flow. We are the only clinic in Oakville that performs shockwave under a WSIB claim.
  3. Active Release Therapy – ART is like a deep tissue massage that breaks up adhesions and scar tissue.
  4. Exercise and Rehabilitation Exercises – Each patient will  get a personalized rehab program for their specific injury!

Shockwave is the gold standard for most chronic conditions. Dr. Steve is one of a handful of practitioners in Ontario who perform Shockwave Therapy on WSIB patient

Depending on your injury the patient will than get a chiropractic adjustment or a session of Instrument Assisted Soft Tissue Therapy. Most WSIB injuries are treated 2 x a week for 4 weeks.
