Dry needling is a treatment technique used by some physiotherapists and other appropriately trained healthcare professionals for certain muscle, tendon and musculoskeletal pain problems.

If you have ever searched for dry needling in Oakville, you have probably seen it described as everything from a treatment for muscle knots to a technique that “releases” trigger points.

The real answer is more interesting.

Why is it called dry needling?

It is called “dry” because nothing is injected through the needle. There is no medication, cortisone, anesthetic or other substance.

Is dry needling the same as acupuncture?

Is dry needling the same as IMS?

IMS, or intramuscular stimulation, is related to dry needling, but the terms are not always interchangeable. Gunn IMS is a specific approach to intramuscular stimulation.

Does dry needling hurt?

What Is Dry Needling?

Dry needling is a musculoskeletal treatment technique involving the insertion of a very thin solid filament needle through the skin.

The needle may be directed toward a painful or sensitive muscle, trigger-point region or another selected musculoskeletal tissue based on the patient’s assessment.

One of the most important things to understand is that dry needling is not an injection.

Nothing comes out of the needle.

There is:

  • no cortisone
  • no anesthetic
  • no medication
  • no fluid
  • no injectable substance

The needle itself provides the stimulus.

This separates dry needling from procedures such as medical trigger-point injections, where medication or anesthetic may actually be injected into the tissue.

Dry needling may be one component of a larger Oakville physiotherapy program that also includes exercise, mobility, strengthening and hands-on treatment.


What Is a Trigger Point or Muscle Knot?

People usually call them muscle knots.

Healthcare professionals often use the term myofascial trigger point.

A trigger point is generally described as a sensitive area associated with a tight or tender region of skeletal muscle.

Pressing the area may reproduce the patient’s normal symptoms.

Sometimes the discomfort stays directly under the clinician’s finger.

Other times the pain may be felt somewhere else.

The painful location and the actual contributing tissue may not be identical.


What Causes Muscle Knots?

There is rarely one single explanation.

Muscles can become tender, sensitive or guarded after:

  • repetitive activity
  • new exercise
  • prolonged computer work
  • sports
  • lifting
  • reduced movement
  • an injury
  • muscle fatigue
  • increased training volume
  • poor recovery
  • sustained positions
  • pain coming from another structure

Sometimes the muscle itself is primarily irritated.

Sometimes it is reacting to something else.

That distinction matters.

A tight upper trapezius muscle may be part of a simple muscular neck pain problem.

But neck and shoulder symptoms can also involve:

  • cervical joints
  • discs
  • irritated nerves
  • the rotator cuff
  • the shoulder joint
  • arthritis
  • previous injury

Treating every sore muscle as an isolated “knot” misses the bigger picture.


How Does Dry Needling Work?

This is where the internet gets a little carried away.

Dry needling is often described as if the needle physically finds a muscle knot, destroys it and instantly returns the muscle to normal.

The actual physiology is more complicated.

Dry needling creates a mechanical and sensory stimulus within the tissues being treated.

That stimulus may influence several processes.

1. Changes in Pain Sensitivity

Needle stimulation activates sensory receptors within the tissue.

The nervous system then processes that information locally, through the spinal cord and within the brain.

This may temporarily change the way pain signals are processed.

2. Changes in Muscle Sensitivity

Some patients notice that a previously tender muscle feels less sensitive following dry needling.

That may make movement easier or reduce the discomfort associated with using the muscle.

3. Local Muscle Responses

A needle entering a sensitive region of muscle may sometimes cause the muscle to twitch briefly.

This is known as a local twitch response.

4. Changes in Movement

If pain and muscular guarding decrease, a patient may temporarily be able to move more comfortably.

That can create a useful opportunity to perform:

  • mobility exercises
  • strengthening
  • corrective movement
  • rehabilitation
  • sport-specific loading

This is one reason we view dry needling as a possible addition to physiotherapy rather than as a replacement for rehabilitation.

5. Local Circulatory Effects

Needling can produce temporary changes in blood flow around the treated area.

That does not mean dry needling literally “flushes toxins” from the muscle.

It simply means that a local physiological response occurs after the tissue is stimulated.


What Is the Local Twitch Response?

One of the more unusual sensations during dry needling is the local twitch response.

This is a brief involuntary contraction of part of a muscle after the needle stimulates a sensitive area.

Patients describe it in all sorts of ways:

  • a jump
  • a quick cramp
  • a tiny spasm
  • an electrical feeling
  • the muscle suddenly kicking

It typically lasts a fraction of a second.

Some dry-needling methods intentionally try to produce a twitch response.

Other techniques do not.

Most importantly:

Getting a twitch does not automatically prove that the muscle has been fixed.

The clinically useful question is whether the patient’s:

  • pain
  • movement
  • strength
  • function
  • activity tolerance

actually improves.

A spectacular muscle twitch is much less important than whether the patient can turn their neck, lift their arm or walk more comfortably afterward.


Dry Needling vs Acupuncture

This is probably the most common question people ask when searching for dry needling in Oakville.

Both techniques use extremely thin needles.

There can also be overlap in where needles are placed.

However, dry needling and acupuncture may use different clinical frameworks when deciding:

  • which areas to needle
  • why the area is being needled
  • how the needle is stimulated
  • what condition is being addressed
  • what other treatment accompanies needling

Dry needling used within musculoskeletal rehabilitation is generally directed toward pain, movement problems and specific tissues identified during the physical assessment.

Acupuncture encompasses a wider range of clinical approaches.

Some acupuncture systems are based on traditional Chinese medicine concepts.

They are related, but they do not necessarily mean exactly the same thing.

Dry Needling

Dry needling is a broad description for using solid filament needles as part of musculoskeletal treatment.

This approach specifically targets areas believed to be associated with painful muscle trigger points.

IMS

IMS stands for intramuscular stimulation.

The term is sometimes used loosely as another description for dry needling.

Gunn IMS

Gunn IMS is a particular system developed around the work of Canadian physician Dr. Chan Gunn.

It traditionally places greater emphasis on possible neurological or neuropathic contributions to persistent musculoskeletal pain.

Therefore:

Not every dry-needling treatment should automatically be called Gunn IMS.


Dry Needling vs Trigger Point Therapy

Traditional trigger-point therapy generally uses pressure from:

  • fingers
  • thumbs
  • hands
  • massage techniques
  • soft-tissue tools

to stimulate sensitive muscular areas.

Dry needling uses a needle.

They may sometimes be trying to influence similar painful muscular regions, but the stimulus is different.

Some patients prefer manual trigger-point treatment.

Others respond well to needling.

Some patients benefit most from exercise and do not need either one.

Dry needling is also commonly associated with shoulder rehabilitation.

Muscles that may become painful or sensitive around the shoulder include the:

Dry Needling for Tennis Elbow

Tennis elbow is also known as:

  • lateral epicondylitis

Plantar fasciitis causes pain around the bottom of the heel and arch.

The problem can be influenced by the entire lower leg and foot complex.

However, new calf pain requires proper assessment.

Calf pain can occasionally represent problems such as:

  • muscular strain

  • vascular conditions

Sudden unexplained calf swelling, warmth or redness should not simply be treated as a muscle knot.


Dry Needling for Sciatica

Dry needling may sometimes help muscular pain associated with a sciatica-type presentation.

That does not mean a needle inserted into the glute or piriformis muscle physically fixes a compressed lumbar nerve.

Sciatica can arise from irritation of lumbar nerve roots associated with conditions such as:

  • disc herniation
  • spinal stenosis
  • inflammation
  • other neurological problems

Some people also have significant gluteal muscular pain alongside their nerve symptoms.

Dry needling may occasionally be directed toward the muscular component.

It does not remove a disc herniation or open a narrowed spinal canal.

Dry Needling for Tendon Problems

Dry needling is also discussed in relation to several tendon conditions.

The important point is that a tendon problem is not simply a muscle knot.

Tendons need to tolerate load.

Even if dry needling reduces pain around a tendon, rehabilitation may still require:

  • progressive strengthening
  • load modification
  • gradual return to activity

  • improving the capacity of the muscle-tendon unit

Dry Needling Is Usually an Addition to Rehabilitation

This is probably the most important thing to understand about dry needling.

Imagine somebody develops shoulder pain after increasing their tennis or pickleball volume.

Their rotator cuff has become irritated.

The surrounding muscles become sensitive.

Dry needling may reduce some of that muscular sensitivity.

Excellent.

But the needle has not automatically restored:

  • rotator cuff strength
  • shoulder endurance
  • tendon capacity
  • movement tolerance
  • sports-specific conditioning

Those still need to be rebuilt.

A useful way of thinking about dry needling is:

Reduce sensitivity → improve movement → restore strength → rebuild capacity

rather than:

Needle the painful area repeatedly forever.

This is why dry needling generally makes the most sense as one possible component of a broader physiotherapy and rehabilitation program.


What Dry Needling Does Not Do

There are a lot of dramatic claims about dry needling online.

Here are some important distinctions.

Dry Needling Does Not Inject Medication

Nothing is injected.

The needle is solid.

Dry Needling Does Not Automatically Break Up Scar Tissue

A needle produces a mechanical stimulus.

That is different from claiming that the needle physically destroys or dissolves scar tissue.

Dry Needling Does Not Flush Toxins Out of Muscles

Muscle pain is not generally caused by mysterious toxins trapped inside a knot.

A Twitch Does Not Mean the Muscle Has Been Permanently Released

A twitch is a physiological response.

It is not proof that the entire problem has been corrected.

More Needles Are Not Automatically Better

Treatment dosage varies.

More needles do not automatically mean:

  • more effective treatment
  • faster recovery
  • better outcomes

More Pain Does Not Mean Better Treatment

Some dry needling can be uncomfortable.

That does not mean the treatment should become a pain-tolerance contest.

Dry Needling Does Not Replace Exercise

If weakness or reduced physical capacity contributes to the problem, strengthening still matters.

That is particularly important with conditions such as rotator cuff problems, tennis elbow and low back pain.

Dry Needling Does Not Cure Every Chronic Pain Condition

Persistent pain can involve much more than one muscle.

The nervous system, sleep, stress, physical capacity, previous injury, activity and other health factors may all contribute.


What Happens During Dry Needling?

Exact treatment techniques vary, but a typical process may look something like this.

Step 1: Assessment

The clinician first determines whether dry needling is appropriate.

An assessment may include:

  • medical history
  • injury history
  • pain location
  • range of motion
  • strength testing
  • neurological testing when indicated
  • palpation
  • functional movement
  • activity history

This helps determine whether the pain actually appears to have a muscular component.

Step 2: Screening

Because dry needling penetrates the skin, the clinician should consider relevant precautions and medical history.

Step 3: Explanation and Consent

Before dry needling, the patient should understand:

The patient can decline dry needling.

Step 4: Positioning

The patient is positioned so the selected muscle or tissue can be accessed safely.

Step 6: Reassessment

After treatment, the clinician may reassess:

  • pain
  • movement
  • range of motion
  • tenderness
  • strength
  • a painful activity

This is important.

Treatment should be judged by whether something useful changed.

Step 7: Rehabilitation

Movement or exercise can then be used to reinforce the improvement.

This may include:

  • mobility
  • stretching
  • strengthening
  • movement retraining
  • progressive loading
  • sport-specific exercises

This is where physiotherapy becomes much more than simply performing a passive treatment.


What Does Dry Needling Feel Like?

Patients describe dry needling very differently.

When the needle initially enters the skin, you may feel:

  • almost nothing
  • a small pinprick
  • mild pressure

If the needle stimulates a sensitive muscular area, you may experience:

  • a deep ache
  • cramping
  • heaviness
  • a brief twitch
  • referred discomfort
  • temporary soreness

Some muscles are much more sensitive than others.

Some people really like dry needling.

Some tolerate it but would not call it enjoyable.

Other people hate needles and want nothing to do with it.

That is fine.

There are many other physiotherapy treatment options.


Does Dry Needling Hurt?

Dry needling can be uncomfortable.

The needles themselves are extremely thin, which means entering the skin can be surprisingly mild.

The stronger sensation tends to occur when a sensitive muscular area is stimulated.

Patients may feel:

  • an ache
  • cramp
  • twitch
  • short sharp sensation

Treatment should remain within an appropriate level of tolerance.

You should always tell the clinician if something feels unusually painful or uncomfortable.


Is Soreness Normal After Dry Needling?

Yes.

Temporary soreness around the treated muscle is relatively common.

Patients sometimes compare it to:

  • a hard workout
  • deep massage
  • a bruised feeling

Soreness often settles over the following day or two.

Some patients experience almost no soreness.

Other minor effects may include:

  • bruising
  • small amounts of bleeding
  • stiffness
  • fatigue
  • light-headedness

Can You Exercise After Dry Needling?

There is no universal rule that every person must avoid exercise for exactly 24 or 48 hours after dry needling.

The answer depends on:

  • the injury
  • the muscle treated
  • how sore you feel
  • the activity you plan to perform
  • the intensity of the activity

Gentle movement is often reasonable.

In fact, movement and exercise may be an important part of what comes next.

If the treated area becomes quite sore, performing a maximal workout for that exact muscle immediately afterward may not make much sense.

Activity should be matched to the diagnosis and your individual response.


Dry Needling Side Effects

The more common side effects of dry needling are generally minor.

They may include:

  • temporary soreness
  • bruising
  • minor bleeding
  • temporary increase in pain
  • fatigue
  • light-headedness
  • feeling faint

These effects generally resolve without significant treatment.


Dry Needling Risks

Dry needling is invasive, so it is not accurate to say there are absolutely no risks.

Less common but potentially more serious complications may include:

  • infection
  • nerve irritation or injury
  • larger hematoma
  • injury to deeper structures
  • pneumothorax

A pneumothorax occurs when air enters the space surrounding a lung.

This is one reason that needling around areas such as the chest, ribs, thoracic spine and upper shoulder requires excellent anatomical knowledge and appropriate technique.

Serious complications are uncommon.

But informed healthcare means discussing the fact that they are possible.


Who Should Be Cautious With Dry Needling?

Dry needling may require additional precautions or may not be appropriate in certain situations.

Tell your healthcare provider if you:

  • take blood-thinning medication
  • have a bleeding disorder
  • are pregnant or may be pregnant
  • recently had surgery
  • have an active skin infection
  • have significant immune suppression
  • have altered sensation
  • have a severe needle phobia
