How Dr. Jenn Uses Acupuncture for Wrist Pain

Dr. Jenn uses an anatomy-based approach to acupuncture.

There is no single group of “wrist pain acupuncture points” that gets used on every patient. Where the pain is located, what movements reproduce it and which tissues are involved determine where treatment is directed.

Depending on the examination, acupuncture may be used around selected muscles of the forearm, wrist and hand. Tight or painful forearm muscles are particularly important because many of the tendons controlling the wrist, fingers and thumb begin much farther up the forearm before travelling into the hand.

Gentle electrical stimulation can also be connected to selected acupuncture needles when appropriate.

The goal is to help decrease pain sensitivity, reduce muscular guarding and improve comfortable movement while the actual cause of the wrist pain is addressed.

Wrist Pain and Inflammation

Patients frequently describe wrist pain as feeling “inflamed.”

Sometimes inflammation is genuinely part of an acute tendon or soft-tissue injury. In other patients, particularly with longer-standing wrist problems, ongoing pain does not necessarily mean that large amounts of active inflammation are still present.

Repetitive loading, tendon irritation, muscular tension, nerve sensitivity and reduced tolerance to gripping or wrist movement can all contribute to persistent symptoms.

Acupuncture is used to help calm painful and sensitive tissues and reduce muscular tension around the injured area. Dr. Jenn does not simply assume that every painful wrist needs to have “inflammation removed.”

The underlying diagnosis still matters.

Numbness and Tingling — Carpal Tunnel Syndrome

Carpal tunnel syndrome involves irritation or compression of the median nerve as it travels through the wrist.

Patients often describe numbness, tingling, burning or altered sensation involving the thumb, index and middle fingers. Symptoms can become particularly noticeable while driving, using a phone or sleeping.

Because carpal tunnel is a nerve problem, it should not simply be treated as generic wrist pain.

Pinky-Side Wrist Pain — TFCC Injuries

Sharp pain on the little-finger side of the wrist can involve the triangular fibrocartilage complex, better known as the TFCC.

Patients frequently notice TFCC pain while gripping, twisting, pushing themselves out of a chair, opening a lid or loading the wrist in certain positions.

That is very different from the numbness and tingling usually associated with carpal tunnel syndrome.

Thumb-Side Wrist Pain — De Quervain’s

Pain where the thumb meets the wrist commonly brings De Quervain’s tenosynovitis into the discussion.

This condition involves irritation of tendons travelling along the thumb side of the wrist. Texting, repetitive gripping, lifting, computer use, sports and repeatedly picking up a baby can all place significant stress through the area.

It is sometimes called Text Thumb, Gamer’s Thumb or Mother’s Wrist.

When acupuncture is appropriate, Dr. Jenn can target selected muscles and tissues of the forearm associated with thumb and wrist movement rather than simply placing needles directly into the most painful spot.

Wrist and Forearm Overuse

Not every painful wrist fits neatly into carpal tunnel, TFCC or De Quervain’s.

Repetitive gripping, weight training, racquet sports, golf, computer use, manual work and other repetitive activities can irritate muscles and tendons throughout the forearm.

Patients sometimes point to their wrist as the problem when much of the muscular tenderness is actually several centimetres farther up the forearm.

That is one reason Dr. Jenn examines the entire area rather than treating only the exact spot where the patient feels pain.

Acupuncture for Wrist Pain During Pregnancy

Pregnancy deserves its own discussion because wrist and hand symptoms are relatively common during pregnancy and after delivery.

Fluid retention during pregnancy can increase pressure around the wrist and contribute to numbness and tingling associated with carpal tunnel syndrome. Pregnancy and early motherhood can also place substantial mechanical stress on the thumb and wrist from repeatedly lifting and holding a baby.

De Quervain’s is commonly referred to as Mother’s Wrist for exactly this reason.

Can Acupuncture Be Used to Relieve Wrist Pain?

Yes, acupuncture can be used as one treatment option for selected causes of wrist pain.

The more useful question is what is causing the wrist pain?

Acupuncture used for painful and tight forearm muscles is different from treating someone whose primary complaint is numbness from median nerve compression. A TFCC injury is different again.

Dr. Jenn first determines which structures appear to be involved and then decides whether acupuncture makes sense as part of treatment.

Acupuncture is particularly useful as a treatment option when muscular tension, pain sensitivity and irritated soft tissues are contributing to the patient’s symptoms.

It is not a replacement for diagnosing the injury.

Acupuncture Does Not Replace Wrist Rehabilitation

Reducing pain is useful, but a wrist also needs to tolerate normal loading again.

Patients who hurt every time they grip, lift or rotate the wrist often need progressive rehabilitation after the more irritable stage begins settling down.

Strengthening the wrist and forearm, improving grip tolerance and gradually returning to normal activities can be extremely important with persistent wrist problems.

Acupuncture can make treatment and movement more comfortable, but Dr. Jenn does not consider temporary pain reduction the entire goal.

The wrist needs to work again.

Acupuncture for Wrist Pain vs Complete Wrist Pain Treatment

What Happens During Your First Appointment?

Dr. Jenn starts by finding out exactly what the wrist is doing.

She will want to know:

  • Whether symptoms travel into the hand or forearm
  • Whether there is numbness or tingling
  • Which fingers are affected
  • Whether gripping or twisting causes pain
  • Whether the problem started after an injury
  • Whether work, sports, texting or computer use aggravates it
  • Whether symptoms wake you at night
  • Whether pregnancy or recently having a baby could be contributing

She can then assess the wrist, hand and forearm and determine which movements and tissues reproduce the symptoms.

If acupuncture is appropriate, treatment can be directed according to those findings.

Where Are Acupuncture Needles Placed for Wrist Pain?

There is no universal answer.

Depending on the problem, selected needles may be placed around muscles of the forearm, wrist or hand.

A patient with thumb-side De Quervain’s pain should not automatically receive the same treatment as somebody with pinky-side TFCC pain or somebody experiencing numbness from carpal tunnel syndrome.

Some patients are surprised that several important treatment areas can be located in the forearm rather than directly on the wrist.

That is because the muscles controlling many wrist and hand movements originate farther up the arm.

Does Wrist Acupuncture Hurt?

Most patients tolerate acupuncture very well.

Acupuncture needles are extremely thin. Patients may feel a brief prick during insertion followed by pressure, heaviness, an ache or occasionally a quick muscular twitch when a sensitive area is treated.

Treatment does not need to be aggressively painful.

Dr. Jenn adjusts needle placement and treatment intensity according to the individual patient.

How Many Acupuncture Treatments Does Wrist Pain Need?

There is no standard number.

A recent forearm overuse injury is very different from months of carpal tunnel symptoms or a structural TFCC injury.

Dr. Jenn monitors things that actually matter: pain, grip strength, numbness or tingling, ability to type, sleep, lifting tolerance, exercise and normal daily activities.

If the wrist is not progressing as expected, the diagnosis and treatment plan need to be reconsidered rather than simply repeating acupuncture indefinitely.

When Wrist Pain Needs Further Investigation

Wrist pain should be properly assessed when symptoms persist, interfere with work or sleep, repeatedly return or prevent normal gripping and lifting.

Further investigation can also become important when there has been significant trauma, substantial swelling or bruising, loss of movement, progressive weakness, persistent numbness or tingling, or concern for a fracture or significant ligament injury.

Acupuncture is not used as a substitute for appropriate medical investigation when the presentation requires it.
